  1. From your ProviderSuite Web app Click on the Reports Tab
  2. In the Sub Tab Click on Appointments
  3. In the Report drop down menu select "Appointment List by Patient"
  4. Enter your date range in the Parameters Filter
  5. Click the Run Report button to generate results
