How to print 1500 Claim Forms

How do you print out 1500 Claim Forms to send to the insurance carrier?

Answer

To print out a claim(s), first, you have to make sure you have clicked the Save Claim-Release button on all of the claims that you want to print. If claims are in a status of Pre-Released, For Review, For Client Review, For Billing Approval, or Imported Charges, you cannot print them yet. 

Note- Printing claims from ProviderSuite onto white paper does not create an official 1500 form. You must separately purchase official 1500 forms and use those forms when printing to create an official paper claim.

Printing Multiple Claims

Follow these steps to print 08/05 or 02/12  1500 forms in bulk from the PracticeAdmin Billing applet. 
  1. Click the Claim-Final Tab.
  2. Under the Print heading on the right side of the screen, click Print Claims. This opens the Print Claims dialog box.
  3. If you do not change any of the default entries when the dialog box opens, you are printing all claims that are in the Awaiting Printing bucket from the Today screen (not advised). Under the Insurance Plan option, clicking the first box checks all of the available insurance plans. Use the filter options and Insurance Plan option box to change which claims will be printed. Note:  Printing the Print Summary Preview details found below the list of claims in the Pending Payer Paper Claim bucket is advised to assist in the filtering process.
  4. Choose either 1500 08-05 or UB 04.
  5. Choose Preview or the Print icon to print the claim forms.

Note- Depending on the number of insurance plans in the practice you may experience some delays in printing. 

Printing Individual Claims

Follow the steps to print 08/05 or 02/12 () forms per claim from PracticeAdmin Billing Manager.
  1. From Today screen, double click the Awaiting Claim File Creation bucket to display a list of claims or Click Claim-General and enter the Claim ID; click GO.
  2. Once the claim is loaded on Claim-General click the Claim-Final Tab.
  3. Under the Print heading on the right side of the screen click Print Claims. This opens the Print Claims dialog box.
  4. Verify that the correct copy of the claim that you want to print is selected.
  5. Choose either 1500 form 02/12 or 08/05.
  6. Click the Print icon to print the claim form.
