Patient Address Report for Mailing Labels

To generate a list of patient addresses for appointment reminder postcards or other mailings, please use the following steps.

Go to ProviderSuite > Reports > Scheduling Reports
Go to the Appointment List by Patient report
Set the report parameters for Facilities, Providers, Appt Type, and Date Range to create the target list of patients
Click the Run Report button
Once the report is generated, hover your mouse over any column heading and a down arrow will appear
Click the down arrow and go to the Columns menu
From the Columns menu, select the columns you would like on the report (Patient, Address, City, State and Zip)
The report will re-generate with the customized columns
Click the Export to Excel button next to the Report drop-down list
