CMS 1500 (02/12) Qualifiers

Qualifiers are codes that are now required for the new 1500 (02-12) claim form in order to state if the date being listed is due to an onset date or some other reason. You can find information at the following website for usage of the new claim form: 
www.nucc.org But the qualifiers are located on the Claim-General tab on the bottom right corner under: Relevant Dates - choose 1500(02-12), then click the Current box- a drop down list of qualifiers will appear and you must choose one then list the date in the area next to it. Then re-release the claim for the changes to be saved. You should then be able to print the claim .
